What not to do!!! There are so many things that we do that can have catastrophic results, the purpose of this sight is to show the results, in the hope someone will NOT repeat these dramatic accidents.

Pressure testing piping systems with air, and what not to do.
This take you to a south american chemcial plant where block valves were used instead of blind flanges.
Make sure you block off piping with blind flanges, not just block valves, prior to pneumatic or high pressure hydrostatic testing of a piping system or vessels; Failure to do so, may lead to catastrophic results.
Segmenting your test of a piping system allows you to test to the precise limits of the various process elements, piping, pumps, tanks, reactors, etc.
This incident occurred at a facility in Brazil during a pneumatic test, when only block valves were used to isolate
the piping. The tank "Launched" and was found on top of the process unit, with extreme damage to facility.
Picture of Foundation after Tank "left the house"
Note the bent tie down bolts.
